Below please find all of the information and forms that you will need for membership!
The two forms that must be submitted to your chapter membership chair prior to initiating a candidate for membership are the Chapter Member Application and the Chapter Member Biographical Data Form #82. These can be xeroxed back to back and will be utilized by the chapter membership chair to complete either an Excel Document that was shared with all chapter membership chairs or a Google Doc: for those of you that have Gmail accounts.
